A Clinical Study of Femoral Shaft Fractures in Children



Abstract
Many kinds of methods in the treatment of femoral shaft fractures in children have been introduced. As a rule, traction method has been accepted as the best one. However, each method of treatment has its advantages and disadvantages in
clinical application.
In choosing the method, many factors must be considered which affect the result of the treatment, such as age of the patient, surgeon¢¥s experience, conditions of skin and surrounding soft tissues, and location and direction of disp¡þlacement of fractured fragments, etc.
However, in many previous reports, overall results were better in children than
in adults since remodeling powers are attributed during the healing process in
children. So anatomical reductions are not always necessary.
Eighty-four cases of fractures of the femoral shaft in children treated at Severance Hospital during the past 7 years and 7 months, were analyzed.
1) In 44.6%, the cases occurred between the ages of five to seven years.
2) They were two times more frequent in boys than in girls.
3) Causes of fractures were either traffic accidents or falling accidents in 76
cases.
4) The middle third of the shaft was involved in 67.9% of cases.
5) The transverse or short oblique type of the fracture comprised 82.1%.
6) Among 70 cases of skin traction, Bryant¢¥s traction was employed in twenty
four, and Russell¢¥ traction in forty-six.
7) Satisfactory results could be obtained in almost all cases of skin traction.
8) Open reductions were performed in five cases, which were mostly malunion.
